Special Ed Profile and Program Pages
The Special Ed Profile or 504 Program page contains student-level program dates and administrative information. Much of the same information can also be viewed on the MIS Summary.
Some fields are populated from finalized forms or meeting activity. Other optional fields may be maintained directly on this page for local program operations.
Program Tab
The Program tab provides a consolidated view of important referral, entry, review, and evaluation dates.
Referral information
Referral information reflects the student's initial evaluation process. When an Initial Evaluation is completed, the resulting referral and evaluation information becomes part of the student's program history.
Plan type and initial entry date
- Plan Type identifies the student's current program type.
- Special Ed. Initial Entry Start Date is the historical date the student first entered Special Education. Once established, it should remain unchanged unless the stored historical date is incorrect.
Review and evaluation dates
The page displays past, current, and expected next Annual Review and Evaluation dates.
When a meeting is open, update the applicable dates through the IEP Manager and meeting forms. Use the Special Ed Profile page to correct historical or calculated dates only after confirming which date is incorrect.
Extended School Year Information
SIRAS separates the primary ESY indicators from the optional operational details:
- ESY Eligible and ESY Enrolled are primary indicators shown on the MIS Summary and in the IEP Manager.
- Additional fields such as ESY School, Classroom, Teacher Name, dates, transportation contacts, and special instructions are available on the Program page.

ESY Teacher may associate the record with a SIRAS user. ESY Teacher Name is a typed text value used for local program information.
Setting and Transportation Tab (Special Education only)
This tab includes student-level setting and transportation information, including:
- Primary Residence and Program Setting;
- percentage of time in General Education, including the available calculation assistance; and
- Special Education transportation information.
When Special Education Transportation is set to Yes, additional transportation information becomes available. Some transportation values may be populated from completed forms, including locally configured bus service request forms.
Services Tab (Special Education only)
The Services tab displays current and historical service-related information, including:
- Current IEP Services and past services;
- Classroom Instruction;
- Accommodations and Modifications; and
- Related or miscellaneous data, including Agency Services, Physical Education, and Other Provider Services.
Service, accommodation, and related-data values are commonly populated from finalized IEP forms. Update the applicable meeting or form when the form is the source of the information rather than changing the stored result without context.
Transition Tab (Special Education only)
The Transition tab provides a consolidated view of transition-related information, including:
- transition regulation indicators;
- Work-Based Experience;
- Classroom-Based Work Exploration;
- Graduation Plan options; and
- Postsecondary Survey responses.
Transition regulation indicators are normally populated through the Transition to Adult form. Postsecondary Survey forms are available from the Document Library under the Tools menu.
Troubleshooting Dates and Unexpected Values
Review the Special Ed Profile when:
- a meeting date was changed repeatedly while a meeting was open and an expected next date is incorrect;
- a student appears on a Meetings Held Late or Previously Overdue list even though the meeting was timely;
- a previous Annual Review or Evaluation date is missing or incorrect; or
- a displayed value does not match the finalized form that should have populated it.
A displayed value may originate from a finalized form, meeting archive, or another Current-data field. Correct the authoritative source whenever possible so the student record remains internally consistent.
